Aims:

  • To provide nurses with the knowledge and skills to assess and manage the problems presented by people who have long-term medical conditions and to enable them to manage and treat these conditions.
  • To encourage the proactive management of people with complex needs.
  • To establish an accepting learning environment that encourages students to explore, discuss and analyse issues relating to the assessment and treatment of long-term conditions.
  • To facilitate the students’ development of self-awareness and promote reflective, non-discriminatory practice, within the clinical setting.
  • To practice in a confident and safe manner in relation to long-term conditions.
  • To provide the opportunity for students to increase their skills in communication and health promotion and to enable them to educate patients, families and carers on the natural course of diseases.

  • Assessable learning outcomes:
    On completion of the Course practitioners will be able to:-

  • Offer an appropriate consultation and examination to patients with long-term conditions, and treat under patient group directions or prescribe if appropriate.
  • Develop individual care plans and ensure that care is integrated.
  • Enable patients to become more confident in their own treatment of their condition. Demonstrate evidence-based practice by critically evaluating and utilising relevant research findings.
  • Recognise the limits of his/her competence and initiate the procedures for making appropriate referrals.
  • Evaluate clinical audit and its relevance to the assessment of long-term conditions.
  • Demonstrate sensitivity to diverse belief systems and critically analyse issues that may influence cultural expectations of individuals.
  • Evaluate the role of nurses in relation to long-term conditions and developments within the NHS.

  • Additional outcomes:

    Outline content:
    The course will have a strong practice focus and will contain:

  • Consultation skills, to include history taking
  • Principles of examination, focused upon physical examination
  • Principles of management to include, pharmacology, investigations, advice and treatment
  • Accountability and clinical governance
  • Socio/political issues in relation to nursing and long term conditions
  • Critical incident analysis and reflection

  • Brief description of teaching and learning methods:
    Both teacher-centred and student-centred approaches will be employed, depending on a number of influencing factors, e.g.; nature of topic, learning environment etc. Teacher-centred methods will include appropriately guided discussion and demonstrations, case studies, lectures and tutorials. Student-centred methods may include brainstorming; buzz groups, debate-discussion groups, interview, role play, seminars, workshops, clinical practice and relevant visits.

    It is envisaged that the strong androgogical emphasis will ensure that the student will be at the centre of the teaching process in order to develop skills that facilitate lifelong learning.

    Coursework

    A written assignment, approximate word count should be 2,500 words.

    Objective Structured Clinical Examination (OSCE)

    Relative percentage of coursework :

    Assignment and Portfolio 70%
    OSCE 30%

    Penalties for late submission of course work will be in accordance with University policy.

    Examinations: None

    Requirements for a pass: 40%

    Reassessment arrangements:
    For the written assignment the student will be required to resubmit the work within three months following confirmation of the result by the external examiner/examinations office.

    Students unable to demonstrate safe practice in the OSCE will be required to re sit this examination within one month.
